Introducing a new Ponseti Clinic service: Care coordination

26. November 2025

We are very happy to announce the launch of a new Ponseti Clinic service - Care Coordinator.

It is a service that we perceive as very necessary in the long term, because many clients find themselves in situations where the health or social system is confusing, complex or too administratively demanding.

A care coordinator is here to bring all these steps together into one understandable, human and safe process.

Who is the service for?

The service is intended for parents of children, adults and seniors who:

What exactly does the service include?

1) Social counseling

We will help you navigate the entire range of benefits and their conditions - from care allowance, mobility allowance to ZTP/P.

We will explain what you are entitled to, how to apply for benefits and what you need to document.

2) Medical devices and their financing

We will go over with you what aids exist (e.g. carts, verticalization stands, chairs, strollers), what the difference is between individual brands and which are or are not covered by the insurance company.

You will get an overview of prices, quality, options for additional payments and recommendations on where to try the tools.

3) Administrative support

Filling out applications for benefits, applications to foundations, communication with insurance companies, SPC, PPP, schools or social workers is often difficult.

Your care coordinator will help you or fully guide you through these steps.

4) Connecting health and social care

We will help you ensure logical follow-up care - from experts in the region, through rehabilitation or the production of orthoses, to connecting the recommendations of doctors with the real possibilities of the family.

This ensures that care is comprehensive and functional, not fragmented.

5) Support in education

We will advise you on the selection of a suitable school, the procedure for applying for a teaching assistant or personal assistant and communication with educational and psychological counseling services.

We help parents with arguments so that their child has conditions that meet his needs.

6) Navigation in regional services

We will look for early care, respite services, patient organizations or support programs in your area.

Thanks to this, you will get a network of experts that you can rely on in the future.

How does the service work?

The first step is an introductory 30-minute consultation, during which we will map out your situation together, define the main needs and determine concrete steps.

You can have a consultation in person at the Ponseti Clinic or online.

Subsequently, the Care Coordinator will help you implement the individual steps, connects experts, explains recommendations, helps with documents and is available to you continuously.

The goal is for your situation to become simplified, clearer and lead to a long-term stable solution.

Possibility of support through the Anežka Foundation

We realize that the Care Coordinator service is often used by people who are already actively looking for help, and it can be difficult for them to handle all the steps financially.

That is why we greatly value the cooperation with the Anežka Foundation, which allows clients to request a financial contribution to pay for this service.

Thanks to this support, the service is available even to those who need it the most - and we sincerely thank the Anežka Foundation for that.

You can order through our reception today.

Ponseti Clinic - We give strength to rise.
