Dear clients, we would like to introduce you to the internal rules of our medical facility, compliance with which is necessary to maintain the current quality of care and proper provision of services. These internal rules are binding for all my clients at the Ponseti Clinic location at Dukelských hrdinů 848/17, 170 00 Prague 7 – Holešovice.
As my patient, you have the following rights
- You have the right to:
- to the free choice of the health service provider;
- to provide services at an appropriate professional level;
- to provide information about expected health performance or service;
- to express free consent to the provision of medical services based on previous information;
- for information about the price of services that are not covered or partially covered by public health insurance and about the method of their payment, if your state of health allows it;
- inspect the medical documentation and take extracts or descriptions from it;
- rights arising from the Act on Health Services No. 372/2011 Coll.
- Minors have the right to the continuous presence of a legal representative, or a person designated by a legal representative, a foster parent or another person based on a decision of a court or other entrusted body.
Making an appointment
- If you want to make an appointment for an examination or medical procedure, it is necessary to make an appointment in advance, through the reception at the telephone contacts listed on our website, unless an appointment is agreed with you directly by the doctor when the services are provided. If you do not have internet access, contact information is located on the entrance door to our facility.
- When ordering, it is always necessary to sufficiently specify which examination you are ordering for, in order to reserve sufficient time for the examination.
- Due to the nature of our facility, due to the busyness or absence of medical staff or other unexpected situation, the appointment time may be postponed, of which you will always be informed in advance.
- Order deadlines are determined by the current capacity and workload of the medical staff.
- The order of patients is determined by the prescribing physician.
- The client is obliged to appear for the examination or procedure on the specified day and time.
- Clients who are insured are required to present a card from the relevant health insurance company. My facility staff may also ask for identification.
- If you cannot come to our facility on the agreed date, you are obliged to inform us of this fact immediately, at least 48 hours before the planned visit and agree on another date. Cancellation of an appointment 48 hours or less before the planned visit is possible only if your health condition does not allow the provision of the service and you document this fact with a medical report.
- V if you do not show up without an excuse and the date of your examination or procedure is missed, this is considered a breach of your duty.
- V in the case of unexcused absence, you are liable to our company for the damage that we incur as a result of not appearing for the ordered examination, and you acknowledge that you will be obliged to compensate for the damage incurred in such a case. In the case of a service paid for directly, i.e. outside the public health insurance system, the amount of damage is equal to the price of the agreed service. If the deadline is not properly canceled in accordance with paragraph 8 of this article, we warn you that the amount already paid for the service, which was not used due to a breach of your obligation, will be offset against the compensation for the damage incurred.
- V in case of repeated absence, this can be considered as not providing cooperation necessary for the further provision of health services and may lead to the termination of the provision of services to your person.
- If there was an infectious viral or bacterial disease (e.g. scarlet fever, flu, COVID-19, etc.) in your household during the 7 days before the planned examination or procedure, or if you were in close contact with a person who had such a disease, always notify us in advance to prevent the spread of the disease to others patients of our facility.
